1. Verify your insurance benefits for nutrition counseling. Call the 800 number on the back of your insurance card and speak to a representative. Visit our Payment and Insurance page and ask these specific questions to find out if your visit will be covered.
2. Schedule an appointment to set up your 60 minute initial visit.
3. Please fill out ALL of the intake documents 24 hours BEFORE your initial visit.  Filling out the documents includes uploading your insurance card (front and back) and photo ID.  After you schedule an appointment, you will receive an email with instructions on how to log on to the HIPAA patient portal and fill out the documents.
4. Please contact us by calling us or emailing us your questions.

During the 60-minute visit, we will discuss all aspects of your lifestyle that contribute to your overall health and develop strategies to meet your personalized goals and needs.

If your insurance will not cover your nutrition visit, the fees are as follows:

Initial visit (1 hour): $195.00

Follow up (50 min): $130.00

You can choose to meet with your dietitian in person located in our office at 455 Main Street in Peckville or you can have a telehealth visit.  If you choose a telehealth visit, you will be directed towards the next steps in setting up your telehealth session. 

Please call your insurance company and confirm if you have telehealth coverage PRIOR to scheduling your visit.
